I use a combination.  I have the &lt;a href="http://www.douglaskarr.com/projects/postpost" rel="nofollow"&gt;PostPost&lt;/a&gt; plugin I wrote... but it still doesn't do what I really need.  Perhaps a release or two in the future?

The rest I simply edit in my theme.
